• Parishioner Registration - This PDF is our main parish registration form for families. You may download and fill it out digitally and print it for submission at the parish office or save it and email is to parishinfo@resurrectionaz.org once complete.

• Flocknote Communications Subscription - You can subscribe to one or more ministries or services of Resurrection and receive regular email or text messages.

INFANT BAPTISMS

• Baptism Registration - For infants to children that are 7 years old. This form will register you so that our baptism coordinator can arrange to meet with you and schedule you for our next baptism class (both required before the actual baptism is celebrated).

• Godparent Registration - For Catholics who are 16 years old or over who have been asked to be the godparent of a child (infant to age 7).

ADULT SACRAMENTS

• Sacraments Registration - For adults who are missing sacraments or would like to be baptized as a Catholic.

• Sponsors - For Catholics who will be sponsoring an adult.
